Immediate Nipple Reconstruction Using the Everted Umbilicus

Abstract

Women considering unilateral immediate or delayed reconstruction may be candidates for abdominally sourced flaps (TRAM, free TRAM, and DIEP) with immediate nipple reconstruction using the everted umbilicus. The umbilical stalk and surrounding skin is included in the harvested flap. Periumbilical perforators provide blood supply to the ipsilateral half of the umbilical stalk, and the contralateral half is supplied by a preserved subdermal plexus. A neo-umbilicus is recreated on the abdominal wall in an esthetically pleasing location.
