Abstract

Uptake of SPECT MPI radiopharmaceuticals within the thin-walled right atrium itself, and more focally within the right auricular appendage, is normal. Avid uptake within the right ventricular free wall, particularly when associated with a flattened septum and D-shaped left ventricle, is consistent with pulmonary hypertension and RV hypertrophy. Prominent uptake within the right ventricular myocardium after exercise stress could signify RV strain and left heart failure due to severe multivessel CAD.
