Pantalar Dislocation

Pantalar dislocations are an extremely rare injury that usually occurs as a result of a high-energy mechanism. A majority of these injuries are open and are associated with a high rate of complications including osteonecrosis, severe post-traumatic arthritis, infection, and need for secondary procedures. Historically, acute talectomy and primary fusion was recommended as the mainstay of treatment; however, recent literature has shifted treatment toward sterilization of the extruded talus and re-implantation with stabilization. The evidence with regard to treatment of these injuries is limited to case reports and case series, and there is currently no consensus on the best treatment of pantalar dislocations.
