Revision total ankle arthroplasty (RTAA) is complex and not without complications. There can be low-grade complications that do not generally imply implant failure or new revision surgeries, or high-grade complications that can worsen the prognosis and survival of the implants. Loosening and sinking of the components is the most feared complication as well as deep infection. We must understand how to prevent them as well as know the optimal treatment to perform if they appear. The functional results are generally satisfactory with great improvement in terms of pain and recovery of previous activity. It is important that the surgery is performed by surgeons who are experts in prosthetic ankle surgery to reduce the rate of complications and improve the survival of the implants. There are also new implants on the market, such as INBONE II total ankle replacement and Invision talar component, which can help us treat cases with bone stock deficit in both the distal tibia and talus.
