Abstract
In the first part of the following chapter, important principles to consider when caring for patients with multiple conditions will be presented and applied to a patient case. These principles include eliciting patient preferences, incorporating the evidence, estimating prognosis, considering treatment complexity and feasibility, and optimizing and prioritizing a care plan. How these principles help to foster a patient-centered approach to care will be explored. In the second part of the chapter, care models that support patient-centered care for patients with multiple conditions and some tools for shared decision-making and polypharmacy management in this patient population will be reviewed.
