Transcatheter Aortic Valve Implantation for Pure Aortic Regurgitation



Transcatheter aortic valve implantation (TAVI) has become an established alternative to surgery in intermediate- or high-risk patients with calcific aortic stenosis. Broad implementation of devices and increased experience led operators to explore the feasibility of TAVI in off-label indication such as pure native aortic valve regurgitation (NAVR). Given technical issues related to the absence of leaflet calcification and aortic root or ascending aorta dilatation, this procedure is typically done on a compassionate basis in patients in whom surgery is deemed prohibitive. Despite several studies reporting a substantial incidence of complications (i.e., post-procedural AR ≥ moderate; need for a valve in valve), recently published data showed encouraging results in patients treated with new-generation prosthesis. New transcatheter valves, specifically designed for treatment of AR, are needed. However, with no dedicated system on the horizon, efforts should be made to improve patients’ selection.
