A Study of Factors Affecting Urinary Citrate Levels
Abstract
Since citrate forms a soluble complex with calcium and has been shown to reduce crystal formation in whole urine in vitro1, hypocitraturia is regarded as a possible pathogenetic factor in calcium urolithiasis. While gross hypocitraturia (in complete distal renal tubular acidosis) is easily demonstrated, defining lesser abnormalities may be more difficult because a number of physiological and pathological factors may affect urine citrate levels. These include renal dysfunction, urine pH, sex and bacterial infection. In this paper an attempt has been made to clarify the role of these factors. The acute effect of oral citric acid loading on urinary citrate has also been studied.
