Diagnostic Strategy for Adult Soft Tissue Sarcomas

Abstract

The optimal diagnostic evaluation of the patient with a soft tissue sarcoma is designed to determine the local and distant extent of the tumor and to establish a diagnosis. If surgery alone is to be the primary mode of local treatment, the local extent of the sarcoma must be determined as accurately as possible. Notable in this regard is the importance of thorough imaging before biopsy. Not only are scintigraphy, arteriography and computed axial tomography less useful in determining local tumor extent after biopsy, but their results may well suggest other pertinent tests needed before biopsy. In this chapter we shall propose a complex diagnostic strategy for the evaluation of soft-tissue sarcomas in adults. This strategy is based on the anatomic behavior of the tumors, and includes the medical history and physical examination, scintigraphic, conventional and computed imaging, and biopsy. The rational application of these tests in an orderly fashion will yield a cost-effective and complete preoperative characterization of the sarcoma in its local and distant extent.
