Infective Endocarditis in Pregnancy

Abstract

Infective endocarditis occurring during pregnancy is uncommon. The incidence has been reported as 0.013% to 1.0% of all pregnancies. 1,2 Although rare, its occurrence during pregnancy may have devastating consequences on both the mother and the fetus. Early diagnosis of this disease in pregnancy is, therefore, critical. Pregnancy affects the cardiovascular system, resulting in increases in cardiac output, blood volume, heart rate, and the common presence of heart murmurs (see Chapter 102). These normal pregnancy changes can complicate the clinical assessment of patients with infective endocarditis. The management of pregnant patients with infective endocarditis involves the same general principles as in the non-pregnant patient. However, special considerations should be made in the diagnostic and therapeutic approach to this disease when it occurs during gestation in order to reduce the risks to the fetus.
