Abstract

A 67 year-old man presented with a history of increasing dyspnoea on exertion, associated with a reduced exercise tolerance of 400 yards. He had previously sustained an inferior myocardial infarction resulting in impaired left ventricular function. Clinical examination revealed a pan-systolic murmur, radiating into the axilla and a laterally displaced apex beat. Moderate bilateral peripheral oedema was also present. Coronary angiography revealed severe three vessel coronary artery disease, with an occluded right coronary artery.
