Samenvatting
Fluorklachten komen veel voor. Oorzaken zijn vaginale infecties, zoals candida of trichomonas, of een verandering van het vaginale microbioom: bacteriële vaginose. In een derde van de gevallen wordt geen microbiologische oorzaak gevonden. Zeldzaam zijn aerobe vaginitis en lactobacillose. In dit hoofdstuk besteden we aandacht aan het vóórkomen en de etiologie van de fluorklachten, met speciale aandacht voor nieuwe inzichten in het syndroom bacteriële vaginose. Anamnese en onderzoek in de huisartsenpraktijk, met en zonder microscoop, komen aan bod evenals de indicaties voor aanvullend microbiologisch onderzoek. Candida en bacteriële vaginose hoeven alleen behandeld te worden bij hinderlijke klachten; spontaan herstel is mogelijk. Bij deze aandoeningen is echter het recidiveren nogal eens een probleem. Trichomonas is een soa, dus behandeling, inclusief die van de partner, is altijd geïndiceerd, net als bij andere soa’s. Als er geen microbiologische oorzaak wordt gevonden, zijn voorlichting en doorvragen belangrijk.
