Stomachirurgie bei Patienten mit kolorektalen Karzinomen

Zusammenfassung

Kolorektale Karzinome gehören zu den häufigsten Indikationen der Stomaanlage. Nach Rektumresektionen mit koloanalen Anastomosen wird eine protektive Stomaanlage empfohlen, wobei sowohl ein Ileo- als auch ein Kolostoma angelegt werden kann. Bei tiefsitzenden Rektumkarzinomen ist die tiefe anteriore Rektumresektion einer andominoperinealen Rektumexstirpation (APE) vorzuziehen. Allerdings ist nicht abschließend klar, ob die Lebensqualität nach intersphinktären Rektumresektionen gegenüber der APE noch als vorteilhaft gewertet werden kann. Eine präventive Stomaanlage vor Radiochemotherapie sollte lediglich Einzelfällen nach kritischer Prüfung überlassen werden. Selbsexpandierende Stents, Resektionen mit protektivem Stoma und alleinige Stomaanlage sind Alternativen im Falle einer akuten karzinombedingten Obstruktion.
