Zusammenfassung
Bauchschmerzen sind das häufigste Einzelsymptom bei pädiatrischen Notfallpatienten. Hauptaufgabe des Erstversorgers ist die nicht einfache Identifizierung von kritischen Situationen, die meist durch einen beginnenden Schock kompliziert sind; für das Erkennen eines solchen bedrohlichen Stadiums ist das Cardiopulmonary Assessment das Standardvorgehen, es priorisiert auch die Maßnahmen für Erstversorgung und Transport im Sinne eines Primary Survey. Das führende Warnzeichen für Kinder mit Bauchschmerzen ist die Tachykardie. Die Kenntnis der verschiedenen Schockformen und der jeweiligen hämodynamischen Besonderheiten sind Voraussetzung für eine strukturierte Behandlung. Kinder mit akutem Abdomen und beginnendem Schock sind einer pädiatrischen Zentrumsklinik zuzuweisen. Assessment und Stabilisierung im Schockraum folgen erneut den Kriterien des Primary Survey, finden im Rahmen einer strukturierten Teamleistung statt und nutzen die Erkenntnisse der klinischen Untersuchung, ergänzt durch Monitoring, Labor und Bildgebung.
