FormalPara Synthese – Verteilung – Abbau – EliminationNach oraler Applikation wird Metamizol als Prodrug rasch zum aktiven Metaboliten 4-Methylamino-Phenazon (4-MAP) hydrolysiert. 4-MAP wird durch Oxidation und N-Demethylierung mit anschließender Acetylierung und Ausscheidung im Urin eliminiert. Durch Dimerisierung kann Rubazonsäure entstehen.
FormalPara Halbwertszeit4-Methylamino-Phenazon: 2–4 Stunden (Plasma).
FormalPara PathophysiologieBei akuter Intoxikation werden beobachtet: Abstumpfung, Koma, Krämpfe, Atemstillstand. Bei chronischer Einnahme können auftreten: Exanthem, Agranulozytose, Thrombozytopenie, Leberschaden.

Rot gefärbter Urin kann auf Rubazonsäure hinweisen, die bei Einnahme von Metamizol (und 4-Aminophenazon) gebildet werden kann.
