Zusammenfassung
Beim CUP (Cancer of unknown primary)-Syndrom handelt es sich um eine histologisch (oder auch nur zytologisch) gesicherte, metastasierte Karzinomerkrankung, deren Ursprungsort, trotz einer Reihe definierter diagnostischer Maßnahmen, nicht gefunden werden kann. Zu diesen Maßnahmen zählen je nach histologischem und immunhistologischem Befund mindestens: die Anamnese, die körperliche Untersuchung (bei Frauen auch gynäkologisch), bei Plattenepithelkarzinom im Kopf-Halsbereich eine HNO-ärztliche Untersuchung, Schnittbilddiagnostik (Computertomogramm von Thorax, Abdomen und Becken als Mindeststandard), eine Laboruntersuchung, eine obere und (bei klinischem oder immunhistologischem Verdacht auf ein Kolonkarzinom) auch) eine untere Endoskopie. Davon zu unterscheiden ist die sehr viel häufigere CUP-Situation, bei der die Metastase die klinische Erstmanifestation darstellt, aber im Rahmen der weiteren Diagnostik der Primarius gefunden wird. Ziel der Diagnostik wird also zu Beginn die Lokalisation des Primarius sein.
