Abstract
Recognition of patients who are at high risk for developing post rhinoplasty airway obstruction is essential. These patients may be at risk because of certain anatomic characteristics or may have a preexisting degree of airway obstruction from previous nasal surgery. Revisional rhinoplasty should address both functional and aesthetic deformities. The author presents illustrative primary and revisional cases. The use of spreader grafts, columellar grafts, batten grafts, and pyriform aperture widening is discussed. The author advocates incorporating these techniques in order to enhance nasal aesthetics as well as function.
