Bei der transkraniellen Gleichstromstimulation (tDCS) wird über an der Kopfhaut angebrachte Elektroden ein kontinuierlicher Gleichstrom verabreicht. Im Gegensatz zu den in der Exkursbox vorgestellten tierexperimentellen Arbeiten zur transkortikalen Gleichstromstimulation ist die transkranielle Gleichstromgabe nichtinvasiv. Da die Elektroden auf der Kopfhaut nicht direkt an der Hirnrinde anliegen, kommt es zu einer Abschwächung der intrakraniell erzielten Stromdichte. Hierbei beträgt die wirksame transkortikale Stromdichte etwa 50% der verabreichten transkraniellen Stromdichte (Rush u. Driscoll 1968). Eine Modellierung der durch die bipolare tDCS beim Menschen induzierten intrakortikalen Stromdichten zeigte, dass eine tDCS mit definierter Stromstärke (2 mA) und Verwendung von Schwammelektroden (25 cm2) kortikal eine Stromdichte in der Größenordnung von 0,1 µA/mm2 induziert (Miranda et al. 2006), was deutlich unter den induzierten Stromdichten am frei präparierten Kortex im Tierexperiment liegt. Das erzeugte elektrische Feld unter der Stimulationselektrode ist dieser Simulationsstudie nach homogen und die Feldstärke nimmt exponentiell mit der Distanz zur Elektrode ab. Dieses Modell berücksichtigte allerdings nicht die regionalen Inhomogenitäten in der Leitfähigkeit der Schädelkalotte und des Hirngewebes (Miranda et al. 2006; Rush u. Driscoll 1968). Im Bereich von Fissuren und Foramina kann es zu inhomogenen, fokal vergrößerten Feldstärken kommen. In diesen Bereichen ist deshalb tDCS zu vermeiden. Des Weiteren ist bisher unzureichend geprüft, wie sich die Feldstärke intrazerebral bei Gewebsübergängen, z. B. zwischen Liquor und Hirnrinde, verhält.
