Additional information

This study began while I was on a sabbatical leave from the University of Redlands. It was continued with support of research grant GB 2735 from the National Science Foundation. I am grateful for the use of facilities at the University of Hawaii and for access to the excellent library of Dr. Maxwell S. Doty. Collection numbers of M. S. Doty are indicated by “D,” mine by “H.” Unless otherwise indicated all “D” collections were by M. S. Doty and all “H” collections by me. Maui, Molokai and Oahu are Hawaiian Islands.
