Both the American Thoracic Society and the European Respiratory Society developed guidelines for the treatment of COPD in 1995.
Lansoprazole and omeprazole were recognized as therapeutically equivalent PPIs, but lansoprazole was selected as the hospital-preferred agent.
Costs and benefits (2000 values) were calculated from the societal perspective. Costs comprized those related to the tai chi program; benefits comprised the avoided costs of hip fracture treatment and the value of morbidity and mortality avoided (estimated using the willingness-to-pay method).
Costs (1997 Australian dollars) were those associated with visits to a general practitioner or specialist, hospitalization and medications.

256 patients (mean age 70 years) with acute myocardial infarction discharged in the 2 years before CHAMPwas implemented (1992 to 1993) were compared with 302 patients (mean age 69 years) discharged in the 2 years post-CHAMP (1994 to 1995).
