Ageing, and Treatment of Hypertension

Summary

There is now clear evidence of benefit from treating hypertension in patients over 60. However, the older patient is vulnerable to the effects of various antihypertensive drugs, which have therefore to be selected with care. Claims of differential efficacy and acceptability of different drugs with age are not well established; at present, the argument favouring a greater effect of ketanserin with age is one of the strongest.

Résumé

Les bénéfices d’un traitement de l’hypertension artérielle chez le patient âgé de plus de 60 ans ne sont plus à démontrer aujourd’hui. Le malade âgé se montre toutefois vulnérable aux effets de certains anti-hypertenseurs, qui doivent donc être choisis avec soin dans cette catégorie de patients. L’existence d’une différence d’efficacité, et de tolérance en fonction de l’ âge, revendiquée pour certains produits, n’est pas totalement établie dans tous les cas. A l’heure actuelle, l’une des mieux confirmées de ces tendances est l’augmentation de l’efficacité de la kétansérine avec l’ âge du patient.

Zusammenfassung

Der Nutzen einer antihypertonischen Therapie bei Patienten über 60 ist inzwischen eindeutig bewiesen. Der Alterspatient ist jedoch empfindlicher gegenüber den Wirkungen verschiedener Antihypertonika, die deshalb mit Sorgfalt gewählt werden müssen. Behauptungen über eine altersabhängige, unterschiedliche Wirksamkeit und Akzeptanz verschiedener Substanzen konnten bisher nicht hinreichend belegt werden; die stärkere Wirkung von Ketanserin im Alter gehört gegenwärtig zu den am besten untermauerten Behauptungen.

Resumen

En la actualidad existe una clara evidencia del beneficio que puede reportar el tratamiento de la hipertensión en pacientes de más de 60 años de edad. Sin embargo, el paciente de edad más avanzada es vulnerable a los efectos de diferentes fármacos antihipertensores, los cuales han de ser seleccionados, por tanto, con sumo cuidado. En el momento actual, no están bien establecidos los argumentos en favor de la eficacia diferencial y la aceptabilidad de los diferentes fármacos en la edad avanzada, siendo uno de los más fuertes el que favorece un mayor efecto de ketanserin en este grupo de pacientes.
