Zusammenfassung
Schmerzhafte Gelenke sind stets mit Entzündungen und Gewebs-umbauten verbunden. Entsprechend der TCM liegt eine Füllestörung des Qi mit erhöhter YangAktivität vor, die durch Aktivierung von Yin kompensiert werden kann. Aus westlicher Sicht entspricht dem eine Überaktivierung des Sympathikus, die durch den Parasympathikus gegenreguliert werden kann. Damit werden Befunde bestätigt, wonach sich Akupunktur besonders eignet, den affektiven Schmerz zu lindern. Durch Stimulation eines Akupunkturpunktes (AP) wird unter zentraler Rückkopplung aus parasympathischen Axonen des Schmerzbereiches, wie auch des AP selbst, über den Parasympathikus ein antiinflammatorisches neuro-im-munes Geschehen aktiviert, durch das die Freisetzung von Noradrena-lin aus Sympathikusaxonen und die Bildung proinflammatorischer Zyto-kine gehemmt wird. Gleichzeitig wird die Synthese schmerzhemmender Endocannabinoide und Opiate aus Immunzellen erhöht. Damit wird eine rückkoppelnde periphere Schmerzverarbeitung angeregt, deren Effektivität vom AChSpiegel durch Akupunktur abhängt. Zusammenhänge mit der zentralen Schmerzverarbeitung werden erläutert.
Abstract
Prominent signs of painful joints are an overactivity of sympathetic axons, inflammation and tissue remodelling. Concerning TCM these pathological conditions are related to an unphysiological “abundance” of Qi with an increased Yang and a decreased Yin. Therefore Yin has to be activated. From a western point of view the complementary Yang and Yin can cautiously be inter-preteted as complementary parts of the vegetative nervous system: sympathicus and parasympathicus. This is confirmed by findings that acupuncture is very suitable for reducing affective pain. Stimulation of an acupoint raises parasym-pathetic activities in the painful area as well as in the acupoint. Acetylcholine (ACh) locally released from parasympathetic terminals and cells of the non-neuronal ACh system decreases norepinephrine from terminal sympathetic axons and supports the synthesis of pain inhibiting endocannabinoids and opioids by immunocytes. These connections activate a peripheral pain reducing feedback loop. Connections to pain processing in the central nervous system are discussed.
