Ariel Pablos-Mendez, Sunil Chacko and Timothy Evans discuss the growing problem of market failures in the development of essential vaccines and drugs for orphan diseases, as well as the public–private mechanisms available to bring the opportunity of new science and technology to bear on diseases of the poor.
