Komplikationen der laparoskopischen Adrenalektomie

Summary

Background: The laparoscopic approach to adrenalectomy has gained wide acceptance and is at the present time considered the ‘gold standard’ approach.

Methods: An analysis of the literature nevertheless reveals dramatic and sometimes tragic complications. They occur during access to the operative field, during exposure of the adrenal area and during dissection of the gland, causing haemorrhages and various organ injuries.

Results: Complications are inherent in any surgical procedure. Respecting standard laparoscopic rules, quickly converting to an open procedure if necessary, and using high-quality materials can minimize their incidence.

Conclusions: In this article, we describe most of the complications that can occur during laparoscopic adrenalectomies, and try to explain their origin as well as ways to prevent them.

Zusammenfassung

Grundlagen: Der Einsatz der Laparoskopie zur Adrenalektomie hat weitgehende Akzeptanz gefunden und wird heute als Goldstandard angesehen.

Methodik: Die publizierte Literatur beschreibt hingegen dramatische und manchmal tragische Komplikationen. Diese ereignen sich beim operativen Zugang und während der Darstellung und Freilegung der Nebenniere. Dabei kann es zu Blutungen und diversen Organverletzungen kommen.

Ergebnisse: Komplikationen können bei jedem chirurgischen Eingriff auftreten. Ihre Anzahl läßt sich jedoch minimieren unter Beachtung standardisierter laparoskopischer Regeln, der Verwendung hochwertiger Materialien und, falls nötig, durch eine schnelle Umwandlung in eine Laparotomie.

Schlußfolgerungen: Wir beschreiben hier die häufigsten Komplikationen, die während einer laparoskopischen Adrenalektomie auftreten können und versuchen, ihren Ursprung zu erklären sowie Möglichkeiten aufzuzeigen, diese zu vermeiden.
