Abstract
Infections following anterior cruciate ligament (ACL) reconstruction are a serious complication of surgery. Preventive measures such as aseptic techniques, systemic single-dose antibiotic, and topical vancomycin application during surgery are of eminent importance. For the successful treatment of a knee infection, early diagnosis is crucial and may be detected by clinical symptoms such as pain and lack of motion. Treatment options include a combination of surgical interventions with joint irrigation and synovectomy as well as intravenous anti-infective therapy. For stages I–III (Gächter classification), arthroscopic therapy is appropriate, while in stage IV or persistent infection, an open approach should be considered. Successful graft-preserving treatment requires a combination of surgical therapy to reduce local bacterial load and adequate, resistance-appropriate anti-infective therapy.
