Die übersehene Kniegelenksluxation mit Gefäßschaden

Zusammenfassung

In diesem Artikel präsentieren die Autoren den Fall einer 18-jährigen Patientin, die sich im Rahmen eines Skatebordunfalles eine initial übersehene Kniegelenksluxation mit Gefäßverletzung zugezogen hat. Es zeigte sich eine vordere und hintere Kreuzbandruptur, eine proximale Innenbandverletzung und ein Verschluss der A. poplitea. Nach Anlage eines distal-origin femoropoplitealen Bypasses erfolgte die Rekonstruktion des hinteren Kreuzbandes mittels eines kontralateralen Hamstringsehnen-Autografts mit Fibertapeaugmentation. Dieser Case Report soll die Notwendigkeit betonen bei Patient*innen mit Luxationsmechanismen des Kniegelenks eine präoperative Gefäßdarstellung durchzuführen.

Abstract

The authors herein report on the case of an 18-year-old female patient who presented with an initially overlooked traumatic knee dislocation with vascular injury caused by a skateboard accident. The patient had suffered rupture of the anterior and posterior curate ligaments, a proximal injury to the medial collateral ligament, and popliteal artery occlusion. After distal-origin femoropopliteal bypass, reconstruction of the posterior cruciate ligament was performed using a contralateral hamstring tendon autograft with fiber tape augmentation. This case report is intended to emphasize the need to perform preoperative vascular imaging in patients with dislocation mechanisms of the knee joint.
