Zusammenfassung
Für die Rekonstruktion des medialen patellofemoralen Ligaments (MPFL) zur Behandlung der Patellainstabilität sind verschiedene Methoden beschrieben. Diese unterscheiden sich vor allem in der Transplantatart und den verschiedenen Fixationstechniken. Die MPFL-Augmentation oder -Rekonstruktion mit autologer Grazilissehne stellt nach unserer Kenntnis die am weitesten verbreitete Technik dar. Zur patellarseitigen Fixierung werden intraossäre, transossäre und Techniken mit diversen Ankersystemen angewandt. Die femorale Fixierung erfolgt üblicherweise mit einer bioresorbierbaren Interferenzschraube. Als interessante Transplantatalternativen werden ein oberflächlicher Quadrizepssehnenstreifen oder der mediale Anteil der Adductor-magnus-Sehne beschrieben. Alle diese Techniken gleichen sich im Ergebnis bezüglich der Knie-Scores (Kujala, Lysholm) und Reluxationsraten. Die Patienten erlangen mit hoher Wahrscheinlichkeit das vorbestehende Alltagsaktivitätslevel wieder. Bei allen Verfahren ist der Erfolg von der Wahrung der anatomischen Landmarken abhängig. Nur unter Berücksichtigung dieser kann die normale Physiologie wiederhergestellt werden. Vor Indikationsstellung zur isolierten MPFL-Rekonstruktion ist es unbedingt notwendig, begleitende, destabilisierende Faktoren und Normabweichungen wie Trochleadysplasie, Achsenfehlstellungen, Patellahöhe sowie pathologische Kraftvektoren innerhalb des femoropatellären Systems zu berücksichtigen.
Abstract
Reconstruction of the medial patellofemoral ligament (MPFL) for the treatment of patellofemoral instability using various techniques has been described. These differ mainly in the choice of graft type and fixation technique. In this article the authors present an overview of well-established techniques. To our knowledge, MPFL augmentation/reconstruction with autologous gracilis grafts is the preferred choice in patella stabilization surgery. For patellar fixation, intraosseous, transosseous and anchor procedures are commonly used. Femoral fixation is usually performed with bioabsorbable interference screws. Interesting graft alternatives include a superficial quadriceps tendon strip or the medial part of the adductor magnus tendon. All these techniques are similar in outcome in terms of scores (Kujala, Lysholm) and redislocation rates. In most cases, patients regain pre-existing levels of daily activity. In all procedures, maintaining anatomic landmarks is key for successful physiological repair. Before isolated MPFL reconstruction, it is crucial to analyze and address any concomitant destabilizing factors, such as trocleodysplasia, malalignment and pathologic aberrations.
