Simultaneous rupture of the anterior cruciate ligament and patellar ligament with an incarcerated bucket-handle tear of the lateral meniscus

Zusammenfassung

Die Kasuistik beschreibt eine kombinierte Kniegelenksverletzung mit Ruptur des vorderen Kreuzbandes (VKB), des Ligamentum patellae (LP) sowie einer Außenmeniskuskorbhenkelruptur. Es handelt sich dabei um eine sehr seltene Verletzung. In der Literatur finden sich drei Fälle mit einer reinen VKB-LP-Rupturkombination, sechs weitere mit zusätzlicher Innenbandbeteiligung, vereinzelt wird eine Beteiligung des Innenmeniskus beschrieben, eine Außenmeniskusbeteiligung konnte nicht gefunden werden. Es besteht in der Literatur kein Konsens bezüglich der Behandlungsstrategie, wobei ein zweizeitiges Vorgehen häufiger angewandt wurde und auch komplikationsärmer zu sein scheint.

Abstract

This article describes an injury with simultaneous rupture of the anterior cruciate ligament (ACL), the patellar ligament (PL), and the lateral meniscus. This is a very rare knee injury. In the literature, three cases describing a pure combination of ALC-PL rupture and a further six cases with additional medial collateral ligament injury are reported. Sporadically, medial meniscus involvement is described; a lesion of the lateral meniscus could not be found. There is no consensus regarding the treatment strategy. In most cases a surgical approach in two steps has been chosen, which seems to be associated with fewer complications.
