Zusammenfassung

In diesem Artikel präsentieren die Autoren den Fall eines 37-Jährigen, welcher sich im Rahmen eines Skisturzes eine Tibiakopfluxationsfraktur zugezogen hat. Dabei zeigten sich ein posteromedialer Spaltbruch sowie ein mehrfragmentär impaktiertes laterales Tibiaplateau mit begleitender Fibulaschaftfraktur. Nach plattenosteosynthetischer Versorgung der posteromedialen Tibia wurde die Fibula adressiert, um hierdurch indirekt das laterale Tibiaplateau mitanzuheben. Nach der Osteosynthese erfolgte eine dynamische Durchleuchtung des Kniegelenkes, um relevante ligamentäre Instabilitäten auszuschließen.

Abstract

The authors present a case of a 37-year-old man who sustained a complex fracture dislocation of the left tibial plateau during a skiing accident. Radiographically, the fracture pattern included a posteromedial fracture with a concomitant lateral split depression and a fibular shaft fracture. Following open reduction and internal fixation of the posteromedial fragment, the authors addressed the fibular shaft fracture in order to improve the reduction of the lateral tibial plateau via the proximal tibiofibular joint. After the final reduction and fixation of the tibial plateau fracture, dynamic stability testing under fluoroscopy was performed to detect potential residual ligamentous instabilities.
