Zusammenfassung
Essstörungen führen zu multiplen Adaptationsmechanismen des endokrinen Systems. Bei Esssucht und resultierender Adipositas kommt es zu einer gesteigerten Leptinbildung in den Adipozyten, welche die Gonadotropinsekretion stört. Bei restriktivem Essverhalten infolge einer Anorexia nervosa (AN) kommt es zu einer verminderten Leptinbildung, welche wiederum eine Störung der Gonadotropinsekretion und in der Folge Zyklusstörungen/Amenorrhö hervorrufen kann. Neben diesen Effekten sind zahlreiche andere Adaptationsmechanismen des endokrinen Systems bekannt, welche als Ziel haben, dem Körper eine Euglykämie zu erhalten. Bei AN kann es zu akuten Störungen im Elektrolythaushalt kommen, welche z. T. lebensbedrohliche Folgen (Syndrom der inadäquaten Sekretion des antidiuretischen Hormons, SIADH) haben. Als Langzeitfolge kann es zu einer Verminderung der Knochendichte und einer Steigerung der Frakturrate kommen.
Résumé
Les troubles du comportement alimentaire déclenchent de multiples mécanismes d’adaptation du système endocrinien. Une hyperphagie incontrôlée et l’obésité qui en résulte entraînent une synthèse accrue de leptine dans les adipocytes, perturbant la sécrétion de gonadotrophines. Un comportement alimentaire restrictif dû à une anorexie mentale (AM) entraîne une synthèse réduite de leptine, ce qui peut interférer avec la sécrétion de gonadotrophines et causer ainsi des troubles du cycle menstruel ou une aménorrhée. À côté de ces effets, on connaît de nombreux autres mécanismes d’adaptation du système endocrinien visant à préserver l’euglycémie du corps. Une AM peut causer des dérangements aigus de l’équilibre électrolytique, ce qui peut avoir des conséquences mortelle (syndrome de sécrétion inappropriée dʼhormone anti-diurétique, SIADH). Les conséquences à long terme peuvent être une réduction de la densité minérale osseuse et une augmentation du taux de fractures.
