Zusammenfassung
Gewicht ist nur ein Symptom der Erkrankung Adipositas, wie beispielsweise Fieber ein Symptom eines Infekts sein kann, aber möglicherweise auch bei einem Schädel-Hirn-Trauma auftritt. Adipositas ist immer noch über den Body-Mass-Index als erhöhtes Gewicht bezogen auf die Körperoberfläche definiert, sollte aber bezogen auf den Krankheitswert eventuell zusätzlich über die Fettfehlverteilung definiert werden. Bei einer Lipodystrophie beispielsweise befindet sich Fett in den inneren Organen und auch im Muskelgewebe. Gleichzeitig fehlt bei einer derartigen Konstellation das Unterhautfettgewebe partiell oder gar komplett, was z. B. zu schweren metabolischen Begleiterkrankungen wie Diabetes mellitus Typ 2, nichtalkoholischer Steatohepatitis (NASH), Pankreatitis usw. führt. Neben dieser Art Fettverteilungstypisierung kann ein Patient mit Adipositas nach den Begleiterkrankungen (Edmonton Obesity Staging System [EOSS]) in ein bestimmtes Stadium (0–3) sortiert werden. Ein höheres EOS ist nach aktueller Studienlage mit einem grösseren Verlust an Lebensjahren und Lebensqualität verknüpft und unterstreicht die Dringlichkeit und rechtfertigt das Risiko einer Therapie. Eine andere Möglichkeit der Typisierung besteht darin, Hunger, Sättigung und Energieverbrennung zu klassifizieren mit standardisierten Fragebögen, radiologischer/nuklearmedizinischer Messung der Magenentleerungsgeschwindigkeit und Messung der basalmetabolischen Rate mittels indirekter Kalorimetrie. Aktuell und auch zukünftig werden sich aus dieser Typisierung nach Fettverteilung, Begleiterkrankung und Hunger/Sättigung/Metabolismus individualisierte Optionen betreffend Ernährungstherapie, Bewegungstherapie, pharmakologische Therapie und sogar chirurgische Therapie ergeben. Erste Studien zeigen bereits, dass der Erfolg betreffend Gewichtsreduktion nahezu doppelt so hoch ausfallen kann, wenn nach einer solchen Typisierung individualisiert therapiert wird. Zudem versprechen die neuen pharmakologischen Therapiekonzepte gastrointestinaler Hormone/Botenstoffe Gewichtsreduktionen über 20 % des Startgewichts. Adipositas ist eine chronische Erkrankung, die multifaktoriell verursacht wird und chronisch therapiert werden sollte. Adipöse Frauen mit einem polyzystischen Ovarsyndrom (PCOS) stellen dabei eine spezielle Subgruppe im Patientengut dar, bei welcher exemplarisch diese individualisierte Therapie betreffend Ernährung, Bewegung und Pharmakotherapie im Detail in diesem Artikel dargestellt werden soll.

Le poids est seulement un symptôme de la maladie qu’est l’obésité, tout comme la fièvre peut être un symptôme d’une infection ou, dans d’autres cas, d’un traumatisme craniocérébral. L’obésité reste toujours définie d’après l’indice de masse corporelle décrivant un poids accru par rapport à la surface corporelle, mais sa valeur de maladie devrait éventuellement inclure aussi la répartition du tissu graisseux. Lors d’une lipodystrophie, par exemple, la graisse est présente dans les organes internes ainsi que dans le tissu musculaire. En même temps, la graisse sous-cutanée manque en partie ou totalement dans cette constellation, ce qui entraîne p. ex. des comorbidités métaboliques sérieuses telles que le diabète sucré de type 2, la stéatohépatite non alcoolique (NASH), la pancréatite, etc. À côté de ce genre de distinction de différents types d’après la répartition des graisses, on peut classer l’obésité d’un patient en stades de 0 à 3 (Edmonton Obesity Staging System [EOSS]) d’après les comorbidités présentes. D’après les études disponibles aujourd’hui, un stade EOS plus élevé est associé à une plus grande perte d’années de vie et de qualité de vie, ce qui souligne l’urgence et justifie le risque d’un traitement. Une autre possibilité de classification consiste à évaluer la faim, la satiété et les dépenses énergétiques à l’aide de questionnaires standardisés, à mesurer la vidange gastrique à l’aide de méthodes radiologiques/de médecine nucléaire et à mesurer le taux métabolique basal par calorimétrie indirecte. Actuellement et à l’avenir, cette classification d’après la répartition des graisses, les comorbidités et les facteurs faim/satiété/métabolisme offrira des options thérapeutiques individualisées pour le traitement nutritionnel, l’exercice physique thérapeutique, la pharmacothérapie et même le traitement chirurgical. De premières études indiquent déjà que le succès de la perte de poids peut presque doubler lorsque le traitement s’inscrit dans l’approche individualisée d’une telle classification. De plus, les nouveaux concepts pharmacothérapeutiques à base d’hormones gastro-intestinales/de messagers chimiques promettent des pertes de poids dépassant 20 % du poids initial. L’obésité est une maladie chronique de causes multifactorielles et exige un traitement chronique. Les femmes obèses atteintes du syndrome des ovaires polykystiques sont un sous-groupe spécifique de patientes pour lesquelles ce traitement individualisé avec ses aspects de nutrition, d’exercice physique et de pharmacothérapie sera présentée en détail dans cet article.
