Zusammenfassung
Der Anstieg der Häufigkeit von Herz-Kreislauf-Erkrankungen als Haupttodesursache der Frau ist seit der Jahrtausendwende dank Optimierung der Akutbehandlung bei kardiovaskulären Ereignissen, v.a. aber durch fortwährende Verbesserung der Prävention rückläufig. Letzteres bezieht sich v. a. auf die Behandlung des Hypertonus, aber im letzten Jahrzehnt auch auf die zunehmende Senkung des Cholesterins. Nahezu unverändert dagegen ist der Anstieg des Diabetes mellitus Typ 2 vor dem Hintergrund einer allgemeinen Gewichtszunahme und Reduktion körperlicher Tätigkeiten. Voraussetzung weiterer Erfolge in der Prävention der Herz-Kreislauf-Erkrankungen ist die ausreichend frühe Wahrnehmung von Risiken seitens der Frauen, aber auch der versorgenden Ärzte, insbesondere des Frauenarztes. Neben Lebensstilmodifikationen haben die medikamentösen Möglichkeiten zur Korrektur von Fettstoffwechselstörungen enorm zugenommen. Triebfeder ist die hohe Wirksamkeit der Senkung des LDL-Cholesterins bezüglich der Vermeidung arteriosklerotischer Herz-Kreislauf-Erkrankungen wie koronare Herzkrankheit und Schlaganfall. Blutdruckerhöhungen reagieren gut auf Gewichtsabnahme, Bewegung und Ernährungsweise und sind mit Kombinationen der Antihypertensiva nebenwirkungsarm beherrschbar. Keine kurative Medikation steht allerdings für den Diabetes mellitus Typ 2 zur Verfügung. Das liegt am irreversiblen Untergang des endokrinen Pankreas. Nur im Frühstadium des Diabetes oder besser bei Prädiabetes ist die Änderung des Lebensstils erfolgreich dann aber hochgradig. Aus diesem Grund und wegen der über Jahre asymptomatischen Entwicklung von Arteriosklerose ist die frühzeitige Diagnose von kardiovaskulären Risiken ein so wertvoller Beitrag zur Gesundheit der Frau.

L’augmentation des taux de maladies cardio-vasculaires en tant que cause principale de décès chez la femme a diminué depuis le début du siècle grâce à l’optimisation des premiers soins des attaques cardio-vasculaires, et surtout grâce à l’amélioration constante de sa prévention. Ce dernier point concerne surtout le traitement de l’hypertension et, au cours de la dernière décennie, la réduction croissante du taux de cholestérol. Par contre, l’incidence croissantes du diabète de type 2 dans le cadre d’une augmentation générale du poids corporel et d’une réduction des activités physiques reste inchangée. Pour permettre de nouveaux progrès dans la prévention des maladies cardio-vasculaires, une perception suffisamment précoce des risques est nécessaire chez les femmes, mais aussi chez leurs médecins traitants, dont en particulier les gynécologues. À côté des adaptations de l’hygiène de vie, les options médicamenteuses pour corriger les troubles du métabolisme des lipides ont énormément augmenté. Ce développement est dû à la haute efficacité d’une baisse du taux de LDL-cholestérol pour prévenir les maladies cardio-vasculaires athéroscléreuses telles que les cardiopathies coronariennes et les AVC. L’hypertension réagit favorablement à une perte de poids, à l’activité physique et à une alimentation appropriée, et les associations d’antihypertenseurs permettent de la contrôler avec peu d’effets indésirables. On ne dispose cependant d’aucune médication curative contre le diabète de type 2. Ceci est dû à la destruction irréversible de la fonctionalité endocrine du pancréas . Une adaptation de l’hygiène de vie n’est efficace qu’au stade précoce du diabète ou, mieux encore, au stade prédiabétique, ou elle est alors extrêmement efficace. C’est pourquoi, vu que l’artériosclérose se développe sans symptômes pendant de longues années, un diagnostic précoce des risques cardio-vasculaires est une contribution si précieuse pour la santé de la femme.
