Résumé
Le carcinome urothélial micro-invasif comprend les tumeurs intra-vésicales de stade pT1 envahissant le tissu conjonctif sous-muqueux mais épargnant la musculature propre de la vessie. Son principal facteur de risque est le tabagisme. En cas d’hématurie inaugurale, il est essentiel d’adresser le patient à un urologue afin qu’un bilan cystoscopique et cytologique puisse être établi.
La survie étant tributaire du stade pathologique de la tumeur intra-vésicale, son traitement en découle également. Toutefois, il existe une hétérogénéité dans l’évolution naturelle des tumeurs de vessie micro-invasives rendant le choix du traitement difficile. Pour les tumeurs à bas risque de récidive et de progression, un traitement conservateur par résesection endoscopique de vessie et instillations d’une immunothérapie ou chimiothérapie intra-vésicale est préconisé. Pour les tumeurs à plus haut risque, un traitement par cystectomie radicale peut être envisagé selon les comorbidités et l’état général du patient.
Zusammenfassung
Mikroinvasive bzw. nicht-muskelinvasive Urothelkarzinome sind Blasentumore im Stadium pT1 mit Infiltration des subepithelialen Bindegewebes, aber nicht der Muskularis propria der Harnblase. Der wichtigste Risikofaktor dafür ist Rauchen. Bei Hämaturie als Erstsymptom ist die Zuweisung des Patienten an einen Urologen von zentraler Wichtigkeit, damit der zystoskopische und zytologische Befund erstellt werden kann.
Da das Überleben vom pathologischen Stadium des Blasentumors abhängt, ergibt sich daraus auch die Therapiewahl. Es besteht jedoch eine Heterogenität im natürlichen Verlauf der nicht-muskelinvasiven Blasentumore, wodurch die Therapiewahl erschwert wird. Bei geringem Rezidiv- und Progressionsrisiko des Tumors wird eine organerhaltende Strategie mit endoskopischer Blasenresektion und intravesikaler Immun- oder Chemotherapie in Form von Instillationen empfohlen. Bei Tumoren mit höherem Risiko kann je nach vorhandenen Begleiterkrankungen und Allgemeinzustand des Patienten eine radikale Zystektomie erwogen werden.
