Zusammenfassung
Die Europäische Lebergesellschaft (EASL) hat aktualisierte Empfehlungen zur Therapie der akuten bzw. chronischen Hepatitis C vorgestellt. Hieraus werden die für die österreichische bzw. die europäische Praxis relevanten Empfehlungen extrahiert, die aktuelle Entwicklungen zur Diagnostik einschließlich der notwendigen Screeningmaßnahmen, zu Therapieindikationen und einzuleitenden therapeutischen Maßnahmen sowie zu den zu beachtenden Kontraindikationen berücksichtigen. Die Anwendung verschiedener Fixkombinationen bei unterschiedlichen Genotypen in Abhängigkeit von der Leberpathologie wird umfassend dargestellt. Hierbei wird auch auf die spezifische Situation bei Substanzabusus bzw. bei relevanter Begleitmedikation, nach Lebertransplantation, im Fall einer Hepatitis-C-Virus(HCV)-Rezidiverkrankung und bei Vorliegen von Immunkomplexablagerungen eingegangen. Bei HCV-positiven Organempfängern und Organspendern sowie bei Vorliegen einer Niereninsuffizienz bzw. anderer relevanter Komorbiditäten muss die Anti-HCV-Therapie entsprechend angepasst werden. Gegebenenfalls ist ein gesondertes Therapiemonitoring erforderlich. Die Behandlung von Kindern und Jugendlichen ist auf die spezifischen Bedürfnisse in der entsprechenden Altersgruppe auszurichten. Pangenotypische Regime sind jedoch für Kinder unter 12 Jahren bisher nicht zugelassen. Die Indikation der verschiedenen wirksamen Substanzen in der Primärtherapie und nach möglicherweise auftretendem Therapieversagen ist jeweils individuell und bezogen auf die einzelnen Krankheitskonstellationen und ggf. in Kenntnis einer Resistenztestung zu stellen. Nach erfolgreicher Therapie sollte eine individuell adaptierte Nachsorge durchgeführt werden, um mögliche Progressionen der Lebererkrankung oder Reinfektionen frühzeitig zu erkennen.
Abstract
The European Association for the Study of the Liver (EASL) has presented updated recommendations for the treatment of acute and chronic hepatitis C. From this, the recommendations relevant for Austrian and European practice have been extracted, taking into account current developments in the diagnostic workup including the necessary screening measures, treatment indications and therapeutic measures to be initiated as well as the contraindications to be observed. The use of various fixed combinations for different genotypes depending on liver pathology is comprehensively presented. The specific situations of substance abuse or relevant concomitant medication, after liver transplantation, in the case of hepatitis C virus (HCV) recurrent disease and in the presence of immune complex deposits are also discussed. In the case of HCV-positive organ recipients and donors and in the case of renal insufficiency or other relevant comorbidities, anti-HCV therapy must be adapted accordingly. Separate monitoring of therapy might be necessary. Treatment of children and adolescents should be tailored to the specific needs of the age group concerned. However, pangenotypical regimes have not yet been approved for children under 12 years of age. The indication of the various effective substances in primary therapy and after possible therapy failure must be made individually and take into consideration the individual disease constellations. Where appropriate, resistance testing can be of help. After successful treatment, individually adapted aftercare should be provided in order to recognize possible progression of liver disease or reinfections at an early stage.
