Zusammenfassung
Die fibröse Dysplasie (FD) ist eine seltene, durch Mutation des GNAS-Gens bedingte Skeletterkrankung, bei der normaler Knochen durch fibrös-ossäres Gewebe ersetzt wird. Bei zusätzlichen extraskelettalen Manifestationen wie Hautüberpigmentierungen und endokrinen Störungen liegt ein McCune-Albright-Syndrom (MAS) vor. Eine FGF-23-Überproduktion in den Osteoblasten kann zu renalem Phosphatverlust und Hypophosphatämie führen. Die Erkrankung manifestiert sich meist im Kindes- und Jugendalter. Klinisch kann ein Zufallsbefund im Röntgenbild bis hin zu einer schwer beeinträchtigenden Erkrankung vorliegen mit Skelettschmerzen, Deformierungen und Frakturen. Häufig betroffen sind das proximale Femur und Schädelknochen. Die radiologische Diagnostik umfasst konventionelle Röntgenbilder, CT und MRT. Mittels Skelettszintigraphie kann die Ausdehnung der Erkrankung erfasst werden. Im Labor können die Knochenumbaumarker erhöht sein, eine Hypophosphatämie kann vorliegen und beim MAS sind hormonelle Störungen nachweisbar. Eine weiterführende Diagnostik erfolgt mittels Knochenpathohistologie oder Molekularbiologie. Die Therapie erfolgt symptomatisch mit Analgetika, Antiresorptiva (Bisphosphonate, Denosumab) und adäquater Substitution mit Kalzium, Vitamin D und ggf. Phosphat. Orthopädisch-chirurgische Maßnahmen sind erforderlich zur Korrektur von Deformitäten, Behandlung und Vorbeugung von Frakturen sowie bei Verdacht auf Malignität.
Abstract
Fibrous dysplasia (FD) is a rare skeletal disease caused by mutation of the GNAS gene, in which normal bone is replaced by fibrous-osseous tissue. FD with additional extraskeletal manifestations, such as skin overpigmentation and endocrine disorders, is known as McCune–Albright syndrome (MAS). FGF-23 overproduction in the osteoblasts can lead to renal phosphate loss and hypophosphatemia. The disease manifests itself mostly in childhood and adolescence. Clinically, a random lesion may be present in the X‑ray image up to a severely debilitating condition with skeletal pain, deformities, and fractures. Frequently affected are the proximal femur and skull bone. Radiological diagnostics include conventional radiographs, CT, and MRI. By means of skeletal scintigraphy the extent of the disease can be recorded. In the laboratory, bone turnover markers can be elevated, hypophosphatemia can be present, and hormonal disorders can be detected in the MAS. Further diagnostics are performed using bone pathology or molecular biology. The therapy is symptomatic with analgesics, antiresorptives (bisphosphonates, denosumab), and adequate substitution with calcium, vitamin D and, if necessary, phosphate. Orthopedic surgical procedures are required to correct deformities, to treat and prevent fractures, and if malignancy is suspected.
