Abstract

Purpose of Review

The purpose of this review is to provide a brief background and review the current literature on resuscitative thoracotomy (RT), as well as review the role of endovascular balloon occlusion of the aorta (REBOA) in current resuscitative strategies.

Recent Findings

Modern comprehensive guidelines have been developed for the use of RT. Despite these guidelines, variation still exists. Inappropriate use of the procedure has high societal costs and puts the healthcare provider at risk. Recent advances in technology have contributed to the use of a low profile endovascular balloon to occlude the aorta during resuscitation. The optimal amount of aortic occlusion is under active investigation. Open compared to closed cardiac massage has not shown significant differences in measured physiologic outcomes.

Summary

The use of resuscitative endovascular balloon occlusion of the aorta (REBOA) is evolving. Future investigations on partial aortic occlusion, ischemia reperfusion, and advanced critical care provide opportunities to further refine resuscitative techniques for patients in extremis.
