Riscontro incidentale di ipercaptazione surrenalica alla PET/TC: significato e algoritmo diagnostico

Nella maggior parte dei casi l’incidentaloma surrenalico è un adenoma corticale non secernente, ma talvolta può rappresentare un tumore maligno primitivo o metastatico. Quando il riscontro di incidentaloma avviene durante un’indagine PET/CT con 18F-FDG, la caratterizzazione metabolica della massa surrenalica permette, con elevata sensibilità e specificità, di distinguere le lesioni benigne da quelle maligne. In altri casi, l’integrazione con altre metodiche di imaging TC e/o RMN si rende necessaria. La valutazione iniziale dovrebbe comunque includere indagini ormonali per l’identificazione di lesioni iperfunzionanti.
