Abstract

Purpose of review

At present, penicillin allergy in children is misdiagnosed, leading to undesirable health consequences; thus, a correct diagnosis is needed. De-labelling penicillin allergy strategies in pediatric population is emerging in order to achieve a correct antimicrobial management.

Recent findings

New protocols, specific for pediatric population, are rising from academic societies in order to improve penicillin allergy management in children, so unnecessary tests are avoided.

Summary

This document aims to provide guidance on how to de-label penicillin allergy in children through a correct diagnosis. The workup for penicillin allergy should be based on the characteristics and severity of the index reaction, which determine the indication and risk for performing a drug provocation test and the indication or nonperformance of previous allergy tests.

This article has been recently published and focus on the importance of de-labeling penicillin allergy, especially in children. Considering the last studies published in with the drug provocation test is the most important approach to confirm or ruled out the diagnosis of drug allergy, in immediate and non-immediate adverse reactions, this study summarizes the novelties and the importance of the risk stratification modelling in index reactions in order to performed a complete allergy workup or performing a DPT directly.
