  1. Introduced in several community pharmacies in the Netherlands in 2006 to facilitate targeted and continuous patient-focussed care with chronic drug treatment. It includes structured counselling sessions followed by compliance monitoring. If treatment is stopped, tailored interventions are used to improve compliance and optimise drug therapy.
