Zusammenfassung
Grundlagen
Die hohe Prävalenzrate des Nikotinabhängigkeitssyndroms unter psychiatrischen Patienten zeigt wie wichtig das Thema Raucherentwöhnung ist. Entsprechend der kurzen Aufenthaltsdauer an akutpsychiatrischen Abteilungen (an unserer Abteilung weniger als 2 Wochen), müssen Therapiekonzepte erarbeitet werden, die in einem „arbeitsteiligen“ Therapieverlauf zwischen stationärer und ambulanter Therapie erfolgen. Entsprechend dem „Transtheoretischen Modell von Prochaska und DiClemente“ (TTM) kann ein therapeutisches Vorgehen strukturiert werden.
Methode
An der Abteilung für Erwachsenenpsychiatrie des Universitätsklinikums Tulln wurden Psychoedukationsgruppen zur Raucherentwöhnung durchgeführt und der Abhängigkeitsgrad mittels Fagerström Test für Nikotinabhängigkeit (FTND) ermittelt. Mit Hilfe von visuellen Analogskalen (100 mm VAS) wurde auch die Ausstiegsmotivation vom Tabakkonsum (100 VAS: höchstmögliche Motivation), der Inhalt der Psychoedukationsgruppen (100 VAS: bestmöglicher Inhalt), der Vortragende (100 VAS: beste Bewertung des Moderators), sowie die Verständlichkeit des Vortrags (100 VAS: bestmögliches Verständnis) evaluiert.
Ergebnisse
Von insgesamt 37 befragten Teilnehmern zeigten 34 Patienten (89,2 %) eine mittelgradige bis sehr starke Nikotinabhängigkeit. Die Motivation zum Rauchstopp lag im Median bei 56 VAS, die Motivationsänderung nach der Psychoedukationsgruppe lag bei 67 VAS, der Inhalt der Psychoedukationsgruppe bei 96 VAS, die Verständlichkeit des Vortrags wurde mit 94 VAS und die Qualität des Vortragenden mit 95 VAS bewertet.
Schlussfolgerungen
90 % der PatientInnen wiesen eine mittelgradige bis sehr starke Abhängigkeit auf. Die Qualität der Informationsveranstaltung und des Vortragenden wurden durchwegs positiv bewertet. Die Motivationsänderung dürfte einem Stadium des TTM entsprechen, daher erscheint eine Vernetzung mit extramuralen Angeboten zur längerfristigen Aufrechterhaltung der Motivation unerlässlich.
Summary
Background
In view of the high prevalence of dependent smokers in psychiatric inpatient facilities advice for smoking cessation seems crucial. Due to the relatively short duration of stay in acute psychiatric wards (in our facility < 2 weeks) there is a need for therapeutic concepts that link to outpatient settings. The transtheoretical model by “Prochaska and DiClemente” (TTM) seems suitable to create an appropriate therapeutic concept.
Methods
At the department of adult psychiatry located at Tulln university hospital, Austria, psychoeducational groups for smoking cessation were conducted. Apart from the degree of dependence using Fagerström test for nicotine-dependence (FTND), 100 mm visual analogue scales (VAS) were utilized to evaluate the patients’ motivation for quitting smoking (100 VAS: maximimum motivation), the presenting physician (100 VAS: best performance), the content (100 VAS: best content) and the comprehensibility (100 VAS: optimum understanding).
Results
Out of 37 participants, the majority (89.2 %), showed a moderate to very strong nicotine dependence. The median motivation for smoking cessation was 56 VAS, the median change in motivation 67 VAS, the content 96 VAS, comprehensibility 94 VAS and presenter was rated with 95 VAS.
Conclusions
In general, patients showed high levels of nicotine dependence. The psychoeducational group was predominantly evaluated in a positive way. Individual change in motivation to quit smoking might correspond to a stage in the TTM making a collaboration with outpatient facilities inevitable.
