Abstract
Die Prävalenz des Syndroms Herzinsuffizienz wird in Deutschland weiter zunehmen, sodass die Versorgung dieser Patienten für Kardiologen wie Allgemeinmediziner von immer größerer Bedeutung wird. In der aktuellen Leitlinie der Europäischen Gesellschaft für Kardiologie (ESC) wird der Empfehlungsstand bezüglich der Diagnostik und Therapie zusammengefasst. In der medikamentösen Stufentherapie haben neben Betablockern und ACE-Hemmern/Angiotensin-II (AT-1)-Rezeptorblockern die Mineralokortikoidrezeptorblocker an Bedeutung gewonnen. Sie werden nun generell für symptomatische Patienten mit einer systolischen Ventrikelfunktion (EF) von ≤ 35% empfohlen. Wichtig bleibt, dass die erstgenannten Substanzklassen nach Möglichkeit auf die in den zugrundeliegenden Studien angestrebten Dosierungen auftitriert werden. Die Herzfrequenz als therapeutischer Zielparameter bekam durch die Studien mit Ivabradin neues Gewicht. Ivabradin stellt als Sekundäroption bei Patienten mit Sinusrhythmus und unzureichender Frequenzkontrolle unter Betablockertherapie nun einen zusätzlichen Therapieansatz dar. Für die primär diastolische Herzinsuffizienz gibt es weiterhin keine neuen Therapieansätze.
