Zusammenfassung

Das Kontaktekzem betrifft als Volkskrankheit alle Altersgruppen mit hoher Prävalenz und Inzidenz. Neben einer Einschränkung der Lebensqualität verursacht es erhebliche gesundheits- und sozialökonomische Kosten. Im Wesentlichen können fünf Unterformen, nämlich das irritative Kontaktekzem, das phototoxische Kontaktekzem, das allergische Kontaktekzem mit seinen zwei Sonderformen des hämatogenen und aerogenen Kontaktekzems, das photoallergische Kontaktekzem und die Proteinkontaktdermatitis, unterschieden werden. Die Diagnostik beruht auf einer genauen Erhebung von Anamnese und klinischem Hautbefund sowie der expositionsbezogenen Durchführung allergologischer In-vivo- und In-vitro-Tests. Ist der Kontaktstoff - Irritans oder Allergen - identifiziert, liegt der Schlüssel zum therapeutischen Erfolg in seiner strikten Vermeidung. Die symptomatische Therapie des Kontaktekzems ist immer individuell anzupassen und nach dem Ekzemstadium auszurichten. Topische Glukokortikoide gelten als First-Line-Therapie beim irritativen wie auch beim allergischen Kontaktekzem. Der stets begleitenden Basistherapie mit Hautpflegemitteln kommt eine zentrale Rolle für einen nachhaltigen Therapieerfolg zu. Eine systemische Therapie kommt in Betracht, wenn die topische Therapie nicht wirksam oder nicht durchführbar ist. Dabei sollte der kurzzeitige Einsatz systemischer Glukokortikoide auf ausgedehnte oder klinisch schwer verlaufende akute Kontaktekzeme und Exazerbationen von chronischen Kontaktekzemen beschränkt bleiben. Die Wirksamkeit des Einsatzes neuerer Biologika und Januskinase-Inhibitoren bei Kontaktekzemen wird aktuell in mehreren klinischen Studien überprüft.
