Zusammenfassung
Eine dauerhafte Erhöhung der Eosinophilen über 0,5 × 109/l im peripheren Blut (PB) sollte weitergehende Untersuchungen nach sich ziehen. Die klonale Eosinophilie macht den deutlich geringeren Teil der Eosinophilen aus (< 10 %), der Ausschluss einer solchen Neoplasie ist jedoch prognostisch und therapeutisch relevant. Zum Ausschluss einer klonalen Eosinophilie werden vor allem molekulargenetische Analysen aus dem PB, eine Zytogenetik aus dem Knochenmark sowie die Histologie des Knochenmarks angewendet. Weitaus häufiger ist die reaktive Eosinophilie, deren Ursache Medikamente, Allergien, solide Tumoren, Lymphome, Wurminfestationen, Autoimmunerkrankungen oder das idiopathische hypereosinophile Syndrom (HES) sein können. Aufgrund der mannigfaltigen Organinfiltrationsmuster bei der Eosinophilie sollte je nach Symptomatik des Patienten gezielt nach möglichen Organbeteiligungen (u. a. Herz, Lunge, Gastrointestinaltrakt, Niere, Haut etc.) gesucht werden. Die Diagnose HES wird gestellt, wenn bei persistierender Eosinophilie nach Ausschluss anderer Ursachen eine Organinfiltration mit konsekutiver Dysfunktion diagnostiziert wird. Therapeutisch kommen beim HES orale Kortikosteroide (OSC) zum Einsatz. Dies kann auch in der Differenzialdiagnostik hilfreich sein, da bei Patienten mit klonaler Eosinophilie in der Regel keine Remission durch OCS zu erwarten ist. Bei hohem OCS-Bedarf werden auch andere Immunsuppressiva (z. B. Methotrexat [MTX], Cyclophosphamid) sowie der Interleukin(IL)-5-Antagonist Mepolizumab eingesetzt. Bei der klonalen Eosinophilie stellen Tyrosinkinasehemmer je nach zugrunde liegender genetischer Veränderung die Therapie der ersten Wahl dar.
