Anaphylaxie auf Zusatzstoffe in Nahrungsmitteln

Zusammenfassung

In Nahrungsmitteln kann sich eine Vielzahl von Zusatzstoffen befinden, die mit der Auslösung unterschiedlicher klinischer Bilder in Zusammenhang gebracht werden. Überempfindlichkeitsreaktionen allergischer und nicht allergischer Genese sind vielfach beschrieben, sollen aber insgesamt selten sein. In dieser Übersichtsarbeit wird eine Auswahl von Nahrungsmittelzusatzstoffen präsentiert, für die anaphylaktische Reaktionen beschrieben wurden. Hierzu zählen unter anderem Farbstoffe (z. B. Tartrazin, Karmin, Patentblau), Konservierungsstoffe (z. B. Sulfite, Natriumbenzoat) und Dickungs-/Geliermittel (z. B. Carboxymethylcellulose, Guar, Gummi arabicum). Mögliche diagnostische Verfahren (Hauttest, spezifisches IgE, zellulärer Antigen-Stimulationstest, orale Provokation) werden vorgestellt. Für Betroffene ist die Meidung des Verzehrs durch die in der Europäischen Union nötige Deklarationspflicht (Klassenname, E-Nummer/Bezeichnung) erleichtert. Als Nahrungsmittelallergen hervorgehoben werden müssen konzentrationsabhängig nur Schwefeldioxid und Sulfite. Aufgrund der nur begrenzt zur Verfügung stehenden diagnostischen Verfahren kann nicht ausgeschlossen werden, dass die Häufigkeit der Überempfindlichkeit auf Nahrungsmittelzusatzstoffe unterschätzt wird.
