Zusammenfassung
Hintergrund
Antibiotika belegen in deutschen Verordnungsstatistiken seit Jahren Spitzenpositionen, gleichzeitig verursachen sie im Vergleich mit anderen Arzneimitteln überproportional häufig allergische Reaktionen. Meist kommt es dann zu Symptomen an der Haut wie Exanthemen, aber auch anaphylaktische Systemreaktionen sind möglich.
Material/Methoden
Eine aktuelle Übersicht der wissenschaftlichen Literatur über Allergien auf Antibiotika wurde mittels Recherchen in Medline, Pubmed sowie den nationalen und internationalen Leitlinienregistern und Cochrane zusammengestellt.
Ergebnisse
Allergische Reaktionen werden vorwiegend auf Antibiotika aus der Gruppe der β-Laktame beobachtet, aber auch auf Clindamycin, Sulfonamid-Antibiotika, Fluorchinolone, Makrolide und Tetrazykline. In vielen Fällen unterbleibt eine eingehende Diagnostik. Die anamnestischen Angaben allein sind aber häufig nicht ausreichend zur Diagnosestellung einer Antibiotikaallergie und können zu falsch positiven Beurteilungen führen. Daher werden den Patienten häufig wirksame Antibiotika ungerechtfertigt vorenthalten beziehungsweise sie verweigern deren Einnahme. So geben bis zu 10 % der Bevölkerung anamnestisch eine Penicillin-Allergie an, aber nur bei circa 2 % kann sie mit Haut- und Provokationstests gesichert werden.
Schlussfolgerung
Eine sorgfältige Diagnostik von Allergien auf Antibiotika ermöglicht sowohl bei positivem Ergebnis den sicheren Nachweis der Erkrankung als auch bei negativem Ausfall die Weiterverwendung wichtiger Substanzklassen und hilft, den unnötigen Einsatz von Reserveantibiotika zu reduzieren.
