Zusammenfassung
Hintergrund
Sulfite sind als Konservierungsstoffe und Antioxidantien in der Lebensmittel-, Kosmetik- und pharmazeutischen Industrie sowie weiteren Industriezweigen weit verbreitet. Sie sind bekannt als Auslöser epidermaler Sensibilisierungen und darauf basierenden ekzematösen kontaktallergischen Hautreaktionen, zu deren Nachweis Natriumdisulfit (= Natriummetabisulfit; CAS 7681-57-4) im Epikutantest eingesetzt wird. Unabhängig davon kann bei inhalativer oder peroraler Sulfit-Exposition das Krankheitsbild der Sulfitunverträglichkeit mit Bronchokonstriktion, Urtikaria, kardiovaskulären, intestinalen und/oder lebensbedrohlichen anaphylaktischen Reaktionen auftreten, zu dessen Nachweis Provokationstestungen notwendig sind.
Methoden
Im vorliegenden Review werden die Verbreitung von Sulfiten, ihre chemischen Grundlagen, gesetzliche Regularien und Grenzwerte, Sulfit-assoziierte Erkrankungen und ihre Diagnostik dargestellt, die berufliche Bedeutung von Sulfiten als Kontaktallergene in der von 1968 bis 2015 publizierten Literatur untersucht und die aktuelle Kontaktsensibilisierungshäufigkeit im Kollektiv einer dermatologischen Praxis überprüft. Zusätzlich wurden Einzelfälle inhalativer beruflicher Sulfitunverträglichkeit erfasst.
Ergebnisse
Im 10-Jahres-Kollektiv einer Hautarztpraxis mit 780 getesteten Patienten war eine Reaktionshäufigkeit von 3,1 % (++ oder +++-Reaktion an Tag 4) auf Natriummetabisulfit (1 % in Vaseline) festzustellen, wobei in keinem Fall berufliche Relevanz vorlag. Demgegenüber betrug die Häufigkeit positiver Testreaktionen auf Sulfite in acht größeren Epikutantestkollektiven 1,1 bis 4,5 % der Getesteten. Eine berufliche Relevanz der positiven Testergebnisse lag (je nach Selektionskriterien) zwischen 0 und 100 %, jedoch mehrheitlich (6 von 8 Studien) zwischen 5 und 14 %. In der publizierten Literatur der letzten 45 Jahren sind circa 50 Fälle mit beruflich verursachtem allergischem Kontaktekzem durch Sulfite publiziert, zusätzlich wenige Einzelfälle einer beruflichen Sulfitunverträglichkeit.
Diskussion
Zusammenfassend sind epidermale Sensibilisierungen auf Sulfite selten von beruflicher Relevanz. Trotz der weiten Verbreitung von Sulfiten als Konservierungsstoffe und Antioxidantien spricht die geringe Zahl dokumentierter berufsdermatologischer Fälle dafür, dass nur selten eine berufliche Sulfit-Exposition in krankheitsauslösender Form besteht.
