Zusammenfassung
Der Begriff α-Gal-Syndrom bezeichnet eine neuartige IgE-vermittelte Soforttypallergie, die gegen das Disaccharid Galaktose-α-1,3-Galaktose (α-Gal) gerichtet ist. Die Bezeichnung als Syndrom wird vorgeschlagen, weil eine klinische Relevanz in den drei unterschiedlichen Allergiefeldern Nahrungsmittel, Arzneimittel und Zeckenstiche besteht. Der Schwerpunkt dieses Artikels ist dem Aspekt α-Gal als auslösendes Allergen für eine Nahrungsmittelallergie gewidmet. Kürzlich wurde gezeigt, dass Soforttypallergien auf Schweinenieren und andere Innereien von Säugetieren ins Spektrum des α-Gal-Syndroms gehören. Diese allergischen Reaktionen manifestieren sich als klassische Allergie vom Soforttyp mit einer typischen zeitlichen Latenz unter einer Stunde. Dabei gilt das Phänomen einer verzögert auftretenden Soforttypallergie mit einer Latenzzeit von drei bis sechs Stunden nach Genuss von Säugetierfleisch als pathognomonisch für das α-Gal-Syndrom. Mithilfe des Konzepts der „food dependent exercise induced anaphylaxis“ (FDEIA) lässt sich dieser klinisch unterschiedliche Präsentationstyp erklären. Klinische Beobachtungen und Expositionstestungen zu dieser Konstellation zeigen aber, dass die individuelle Empfindlichkeit von α-Gal-Patienten recht variabel ist und dieser Umstand für das grundsätzliche Verständnis des α-Gal-Syndroms hilfreich sein kann.
