Für die plättchenhemmende Langzeittherapie nach perkutaner Koronarintervention (PCI) scheint Clopidogrel eine bessere Wahl als ASS zu sein.
In der antithrombotischen Nachbehandlung nach PCI mit Stent-Implantation steht zunächst eine zeitlich befristete duale Plättchenhemmung (DAPT) an. Danach wird gemäß Leitlinien-Empfehlung zumeist auf eine unbefristete Monotherapie mit ASS umgestellt. Clopidogrel weist hier aber ein vorteilhafteres Nutzen/Risiko-Profil auf, so das Ergebnis der jetzt beim ACC-Kongress präsentierten HOST-EXAM-Studie. In der Studie waren an 37 Zentren in Südkorea im Anschluss an die DAPT-Phase 5.438 KHK-Patienten auf Clopidogrel (75 mg/d) oder ASS (100 mg/d) randomisiert worden.
Einen primären Endpunkt (Tod, Myokardinfarkt, Schlaganfall, Wiedereinweisung wegen akutem Koronarsyndrom, Blutungen) erlitten in zwei Jahren 152 Patienten (5,7%) unter Clopidogrel und 207 Patienten (7,7%) unter ASS (HR: 0,73, p = 0,0035). Die Bahandlung mit Clopidogrel war sowohl bei thrombotischen Ereignissen als auch bei Blutungen im Vorteil, berichtete Studienleiter Dr. Hyo-Soo Kim vom Seoul National University Hospital. Insbesondere reduzierte es die Risiken für Schlaganfälle (0,7 vs. 1,6, p = 0,002), erneute akute Koronarsyndrome (2,5% vs. 4,1%, p = 0,001) und schwere Blutungen (1,2% vs. 2,0%, p = 0,035).
