Eine 81-jährige Patientin stellt sich mit Verschlechterung ihres Allgemeinzustandes beim Hausarzt vor. Dort fallen blasse Schleimhäute und eine blutgefüllte Blase (2 x 3 cm) am linken Schienbein auf. Eine ischämische Herzinsuffizienz, ein permanentes, normofrequentes Vorhofflimmern und eine chronische Niereninsuffizienz sind bekannt. Die Patientin gibt zusätzlich Teerstuhl seit drei Tagen an. Bei V. a. gastrointestinale Blutung (Hb-Wert 7,1 g/dl) wird die Patientin stationär eingewiesen.
