Zusammenfasung

Funktionelles Sodbrennen ist eine häufige Erkrankung, an die insbesondere dann gedacht werden sollte, wenn das Symptom Sodbrennen nicht oder nicht ausreichend auf eine Standardtherapie mit einem Protonenpumpeninhibitor (PPI) anspricht. Das Krankheitsbild ist nach den Rom-III-Kriterien klar definiert. Zur differenzialdiagnostischen Abgrenzung von der gastroösophagealen Refluxerkrankung (GERD), der nichterosiven Refluxerkrankung (NERD) und dem hypersensitiven Ösophagus sollte eine Endoskopie und idealerweise auch eine pH-Metrie durchgeführt werden. Die Therapie ist im Wesentlichen empirisch, da klinische Studien zum funktionellen Sodbrennen derzeit fehlen. Wichtig für eine erfolgversprechende Behandlung sind in erster Linie die Aufklärung des Patienten, die Identifizierung des führenden Symptoms und das Setzen realistischer Therapieziele.

Abstract

Functional heartburn is a frequent disease which should be considered when heartburn does not respond to proton pump inhibitor (PPI) treatment. Functional heartburn is defined in the Rome-III-criteria. Differential diagnoses like gastroesophageal reflux disease (GERD), non erosive reflux disease (NERD) and hypertensive esophagus are diagnosed employing an endoscopy and a 24h-pH-study. Since clinical trials are lacking, treatment approaches are empirical. Successful treatment includes adequate information of the patient, identification of the cardinal symptom and setting of realistic goals.
