Riassunto
Premesse
. La IV Indagine del Gruppo di Studio Marcatori Miocardici (GdS MM) SIPMeL sull’uso dei marcatori cardiaci in Italia, tenutasi nel 2017, ha inteso rilevare diffusione, caratteristiche operative e governo clinico del POCT in Italia, dove è cambiato il contesto clinico (introduzione della troponina ad alta sensibilità, hs-cTn) e organizzativo (consolidamenti geografici e creazioni di reti diagnostiche hub & spokes).
Metodi
. Nove item, sui 62 del Questionario utilizzato, erano dedicati ai POCT per marcatori miocardici. A essi hanno risposto 92 Laboratori (75,4%) su un totale di 122 rispondenti.
Risultati
. Il 25% dei rispondenti dichiara che sono presenti nella sua realtà organizzativa POCT esclusivamente dedicati ai marcatori cardiaci, in rapporto più alle mutate condizioni organizzative che per abbreviare i TAT e con un aumento di circa 1% per anno negli ultimi 5 anni. Per quanto riguarda le caratteristiche di performance e di governo clinico, la matrice utilizzata è sangue intero (50%) o plasma (50%) e il metodo è omogeneo o comparabile con quello del Laboratorio centrale (LAB) solo nel 30% dei casi. Il POCT è allineato al metodo LAB nel 50%, sotto controllo di qualità interno (CQI) ed esterno (VEQ) rispettivamente nel 99% e 33%, e sottoposto a supervisione da parte del LAB nel 95% dei casi.
Conclusioni
. I dati positivi appaiono essere la diffusione di POCT supervisionati dal Laboratorio, l’utilizzo di matrici adeguate e l’esteso uso del CQI; i dati negativi, viceversa, appaiono essere l’utilizzo di un metodo POCT comparabile con il LAB solo nel 30% dei casi, un allineamento allo stesso solo nel 50% dei casi e le difficoltà per l’uso di VEQ. Data la diversità analitica e diagnostica tra POCT e LAB e tra diversi strumenti POCT, i clinici devono essere informati dal Laboratorio sulle caratteristiche analitiche e diagnostiche dei POCT in uso e clinici e Laboratorio devono scrivere insieme i protocolli di utilizzo degli stessi. Le Società scientifiche devono accompagnare il corretto utilizzo dei POCT per marcatori miocardici.
Summary
Background
. In 2017 Working Group on Myocardial Markers of Italian Society for Laboratory Medicine (GdS MM SIPMeL) made the III survey on the use of cardiac markers in Italy.
Methods
. 122 colleagues answered a 62-items Questionnaire and 92 (75.4%) the 9 specific items about POCT for cardiac markers.
Results
. POCTs dedicated to cardiac markers (usually cTn and CNPs) were present in 25% of surveyed trusts, with a slight increase of 1% per year in the last 5 years, following the implementation of diagnostic networks for acute coronary syndromes (ACS). In POCT settings, the sample was whole blood or plasma (50/50%), the method was comparable to the method of the central Laboratory (LAB) only in 30% and aligned in 50% of cases, quality controls were used in 99% (IQC) and 33% (EQA) respectively, and the POCT instruments/devices, methods and quality controls were supervised by LAB in 95% of cases.
Conclusions
. Encouraging results are the correctness of sample, the use of IQC, and the supervision by LAB; main concerns are the comparability and the alignment of POCT to the LAB method, and the use of EQA. Clinicians should be informed by LAB about analytical and diagnostic characteristics of the POCT and LAB methods in use and clinicians and LAB together should write specific protocols for the use of biomarkers in ACS by LAB and POCT. Scientific Societies should help the correct implementation and use of POCT for cardiac markers.
